NEXT STEPS
Step 1
Authorize Release
If your loved one passed away, or will pass away:
- In a Hospital, Facility, or Home Hospice:
- Simply give our information to the nursing staff. They will contact us when they are ready for us.
- At Home, NOT Under Hospice Care:
- Call 9-1-1. The authorities will come to the home to determine if your loved one can be released to us, or if they will need to go to the Medical Examiner for examination.
Step 2
Submit Vital Information
- Click the button below to submit your loved one's Vital Information. From the information you provide, we will create your documents, including the death certificate.
- We will email them to you for electronic signature
or
- We can set an appointment for you to sign in person at our office.
